If you do not have insurance, or choose not to use your insurance benefits, you have the right to receive a written Good Faith Estimate outlining the expected cost of your services before your appointment.

If your visit is scheduled at least one business day in advance, your estimate will be provided prior to your appointment. You may also request one at any time before scheduling.

If your final bill is $400 or more above your estimate, you have the right to dispute the difference.

Please keep a copy of your estimate for your records.
